Діагностика раку ротової порожнини і ротоглотки, асоційованого з вірусом папіломи людини

Abstract

Мета: визначити ознаки інфікування вірусом папіломи людини (ВПЛ) 16/18-го типу в карциномі ротової порожнини і ротоглотки та розробити алгоритм діагностики ВПЛ-позитивних (ВПЛ+) і ВПЛ-негативних (ВПЛ–) пухлин. Об’єкт і методи дослідження: біоптати пухлин 128 хворих на плоскоклітинний рак ротової порожнини (РРП) та ротоглотки (РРГ). Використано полімеразну ланцюгову реакцію (визначення ДНК ВПЛ 16/18-го типу), імуногістохімічний (визначення білка Е6 ВПЛ 16/18-го типу, білків p16^INK4a і р53) та гістологічний методи (визначення койлоцитозу). Результати: ДНК ВПЛ 16/18-го типу виявлено у 8 (6,3%), койлоцитоз — у 69 (53,9%), одночасну експресію p16^INK4a і Е6 ВПЛ 16/18-го типу — у 60 (46,9%), експресію р53 — у 73 (57,0%) хворих. Показано, що одночасна експресія білків Е6 і р16 є ознакою наявності ВПЛ у пухлині. Чутливість і специфічність визначення койлоцитозу як маркера ВПЛ становлять відповідно 0,83 та 0,72. Розроблено алгоритм виявлення ВПЛ у РРП та РРГ. Висновки: плоскоклітинний РРП і РРГ у 46,9% випадків асоційований з ВПЛ 16/18-го типу. Розроблений алгоритм може бути рекомендований для діагностики ВПЛ+ і ВПЛ− форм злоякісних орофарингеальних пухлин.Objective: tо determine the characteristics of human papillomavirus (HPV) 16/18 types of the oral cavity and oropharynx carcinomas and develop an algorithm for diagnosis of HPV-positive (HPV+) and HPV-negative (HPV-) tumors. Object and Methods: 128 tumor biopsies of patients with squamous cell carcinoma of the oral cavity and oropharynx were investiga ted. Polymerase chain reaction (determination of DNA HPV 16/18 types), immunohistochemical (determination E6 protein of HPV 16/18 types, of proteins p16^INK4a and p53) and histolo gic examination (determination of koilocytosis) were used. Results: HPV DNA 16/18 types was found in 8 (6,3%), koilocytosis — in 69 (53,9%), while the expression of p16^INK4a and HPV E6 16/18 — 60 (46,9%), the expression of p53 — in 73 (57,0%) patients. It was shown that the simultaneous expression of proteins E6 and p16 is a sign of the presence of HPV in the tumor. Sensitivity and specificity determination of koilocytosis as a marker of HPV is respectively 0,83 and 0,72. An algorithm for detecting HPV in carcinoma of the oral cavity and oropharynx was investigated. Conclusions: squamous carcinoma of the oral cavity and oropharynx in 46,9% of cases are associated with HPV 16/18 types. The algorithm can be recommended for the diagnosis of HPV + and HPV− oropharyngeal forms of malignant tumors.
